On 10/03/2007, the firm sent Consignees a Customer Safety Advisory Notice informing them of the two potential issues with the display of reference images used for position correction. Consignees will be informed when a solution to this issue has been validated.
Radiation Protective Eyewear (only involving multifocals. FTC 28 Progress X-Ray Protective Eyewear Protective Eyewear is not sent with any package inserts or labeling. It is sent is plain envelope, bag of box.
Radiation Protective Eyewear does not meet the radiation protection levels stated on the labeling.
Customers were all notified by phone, followed by an Urgent Recall letter dated June 3, 2008. The letter asked that customers examine their stock immediately to determine if they had any of these radiation protective bifocals on hand, discontinue use or distribution of the lot, and set the products aside for further instructions. Also, if customers distributed these items, they needed to contact their accounts and advise them of the recall and have them return their outstanding recall stock to them. The firm will send replacement products in the weeks to follow.
POWER STEERING RACK
IF A DEFECTIVE DRIVING PIN EXISTS IN A VEHICLE AND THE PIN BREAKS DURING NORMAL VEHICLE TURNING, THE NORMALLY AVAILABLE HYDRAULIC SUPPORT FUNCTION WILL NO LONGER FUNCTION AND STEERING WHEEL OPERATION WILL BE LIMITED.

THE PURPOSE OF THIS PART IS TO SPECIFY THE CONTENT AND LOCATION OF, AND OTHER REQUIREMENTS FOR, THE CERTIFICATION LABEL TO BE AFFIXED TO MOTOR VEHICLES TO ADDRESS CERTIFICATION-RELATED DUTIES AND LIABILITIES, AND TO PROVIDE THE CONSUMER WITH INFORMATION TO ASSIST HIM OR HER IN DETERMINING WHICH OF THE FEDERAL MOTOR VEHICLE SAFETY STANDARDS ARE APPLICABLE TO THE VEHICLE.

THE FAILURE OF THE BRAKE LAMPS TO ILLUMINATE WHEN THE BRAKES ARE APPLIED WILL NOT PROVIDE WARNING TO FOLLOWING TRAFFIC AND COULD RESULT IN A CRASH.

SEAT BELT WEBBING
THE SEAT BELT WILL NOT TIGHTEN AROUND THE OCCUPANT AND WILL NOT OFFER RESTRAINT IN THE EVENT OF A CRASH WHICH MAY RESULT IN INJURY.

KING PIN FAILURES
IF A KINGPIN WERE TO BREAK OFF DURING OVER THE ROAD OPERATION, IT COULD RESULT IN THE SEPARATION OF THE TRAILER FROM THE TRACTOR WHICH COULD INCREASE THE RISK OF A CRASH.
DEALERS WILL INSPECT THE KINGPINS FOR LOT/HEAT CODE. ALL TRAILERS IDENTIFIED WITH KINGPINS WITHIN THE SUSPECTED LOT AND HEAT CODE WILL BE REMOVED FROM SERVICE AND SCHEDULED FOR FIELD TESTING. Newport 4-in-1 Cribs and Matching Furniture Recalled for Poisoning Hazard
The red paint which is underneath the black finish paint on some of the cribs and matching furniture exceeds federal lead limits. If ingested by young children lead can cause adverse health effects.
Consumers should immediately stop using the recalled cribs. Destroy and discard the cribs. The firm is no longer in business. A remedy is no longer available.
Durin Race 80/100 Bicycle Suspension Forks Recalled for Crash Hazard
The front bicycle suspension fork can break, causing the rider to crash.
Consumers should immediately stop using bicycles equipped with the recalled suspension forks and contact Magura or visit www.magura.com, to determine if your bicycle fork is included in the recall and, if it is, to receive a free repair.
Jumbo Snowman Snow Globes Recalled for Fire Hazard
When exposed to sunlight, the snow globe can act as a magnifying glass and ignite nearby combustible materials, posing a fire hazard.
Consumers should immediately remove the snow globe from exposure to sunlight and return it to any Hallmark Gold Crown store for a full refund.
Foursquare Hooded Jackets Recalled for Strangulation Hazard
The jackets have a drawstring through the hood, which poses a strangulation hazard to children.
Consumers should immediately remove the drawstring from the sweatshirts to eliminate the hazard, or contact Foursquare for a full refund.
Candle-Powered Carousels Recalled for Fire Hazard
The candle holder placement on the base of the carousels is too close to the structure, allowing the candle flame to come in contact and ignite different parts of the carousel, including the fans, trees and deer, posing a fire hazard.
Consumers should stop using the carousel immediately and return them to the store where purchased for a full refund. Consumers also can mail the product to Gardener's Supply Returns, 5 New England Drive, Essex Junction, VT 05452.
Contessa Office Chairs Recalled for Fall Hazard
The bolts holding the seat to the frame can loosen and separate, posing a fall hazard to consumers.
Consumers should inspect the chair by checking the four bolts that attach the seat bottom to the chair frame. If any of the four bolts is loose or missing, consumers should stop using the chair immediately and contact a Teknion dealer for a free repair.
Alpine Ski Bindings Recalled for Fall Hazard
The heel housing of the bindings can crack, causing the binding to release unexpectedly. This can cause the skier to lose control or fall and suffer injuries.
Consumers should stop using these ski bindings immediately and return them to any authorized Atomic Ski dealer for a voucher toward a new heel component.
Calypso Steel Drums Recalled for Poisoning Hazard
Surface paint on the recalled toy drums contain excessive levels of lead, violating the federal lead paint standard.
Consumers should immediately take the recalled steel drum away from children and return it to Woodstock Percussion for a replacement drum and a $5 credit or payment.

Reinforcement spring may detach from the cannula body resulting in reduced venous blood return flow.
All domestic consignees were notified by a Product Recall letter on 07/15/2008 via FedEx. They were instructed to check their inventory for any affected product and return all unused product from their stock to Edwards. Customers are to contact Customer Service Organization to obtain a Returned Goods Authorization number and replacement product. Once inventory has been verified, the attached form needs to be completed and faxed to Edwards so that customer awareness can be confirmed. International accounts were notified on 07/14/2008.
SERVO-i Ventilator System Maquet Critical Care, Solna, Sweden. The product is intended for treatment and monitoring of patients in the range of neonates, infants and adults with respiratory failure or respiratory insufficiency.
Maquet became aware of potentially defective crimpings at the point connector attached to the cable in SERVO-i and SERVO-s ventilators.
Urgent - Device Correction notices were sent out to all customers on July 7, 2008. The letter informed the customers of the affected product, and associated hazards.
