Wiring a House, 3rd Edition and Wiring Complete, Expert Recalled for Shock Hazard
The books contain several errors in the technical diagrams that could lead consumers to incorrectly install or repair electrical wiring, posing an electrical shock hazard to consumers.
Consumers should immediately stop using the books and return them to the place of purchase for a full refund.
Notifier Fire Alarm Control Panels Recalled for Fire Hazard
The software in the fire alarm system could fail to process an alarm condition which could result in the system failing to respond in the event of a fire.
Distributors should immediately contact the company for replacement software. All known users of have been contacted.
Boy's Reversible Vests Recalled for Choking Hazard
The zipper tabs on these vests fail to meet the children's torques test standards, posing a choking hazard to children.
Consumers should immediately take these vests away from children and return them to the store where purchased for a full refund.

Consignees were notified via Medical Device Recall Notification letter dated 2/13/08 to read the enclosed instructions for use, remove all old instructions for use at the consignee location and replace them with the new instructions for use, and to discard the old instructions for use.

Package Integrity; sterility compromised-Incomplete seals on the nylon bags in which the product is packed.
Each consignee will be contacted by email or telephone followed by a faxed letter. Each consignee was instructed to check their respective inventory for the recalled part and corresponding lot number. They were directed to return the product to the recalling firm.

False negative test results due to Interfering substances (heterophile antibodies) in the patient's blood may produce erroneously low results.
Consignees were notified of the problem via Urgent Medical Device Correction letter dated 12/20/07. The letter provided information on the issues concerning the recall. The firm will revise the package insert to address the potential occurrence of false negative bias. If inconsistencies with Vancomycin level exist, confirm with an alternative test method. Repetitive mechanical stress applied to the titanium case can induce component damage and device malfunction in certain ICD and CRT-D devices implanted subpectorally with the serial number facing the ribs. An Urgent Medical Device Information Update to May 2006 Product Advisory, dated January 04, 2008, was sent to implanting physicians and physicians following implanted devices beginning Monday, January 07, 2008. The letter described a malfunction that can occur when the ICD is implanted subpectorally with the serial number facing the ribs. Devices implanted subcutaneously or in a subpectoral position with the serial number facing away from the ribs are not included in this advisory. The physicians were asked to review records to determine if an affected device was implanted subpectorally. An AP radiograph should be done to determine specific device orientation for subpectoral implants. If a subpectoral implant is in a susceptible orientation, consider repositioning or replacement for physically active patients who regularly need device therapy. Follow patients with susceptible devices at 3-month intervals.
FRONT BRAKE DISC FASTENERS
IF THIS HAPPENED, THE FASTENER COULD WORK LOOSE FROM ITS THREADS. IN SOME CASES INVOLVING MULTIPLE FASTENERS, THE BRAKE DISC COULD LOOSEN, INCREASING THE RISK OF A CRASH.
DEALERS WILL REMOVE THE BRAKE DISC FASTENERS AND REPLACE THEM WITH NEW LONGER FASTENERS. THE RECALL IS EXPECTED TO BEGIN DURING JANUARY 2009. OWNERS MAY CONTACT BMW AT 1-800-831-1117.
CHAIN SPROCKET FASTENERS
IT IS POSSIBLE FOR THE FASTENERS TO WORK LOOSE FROM THE SPROCKET, INCREASING THE RISK OF A CRASH.
DEALERS WILL LOOSEN AND INSPECT THE CHAIN SPROCKET FASTENERS. THOSE THAT HAVE BEEN DAMAGED WILL BE REPLACED. IN ALL CASES, THE FASTENERS WILL BE RETIGHTENED. THE RECALL IS EXPECTED TO BEGIN DURING JANUARY 2009. OWNERS MAY CONTACT BMW AT 1-800-831-1117.

THIS STANDARD REQUIRES PROPER IDENTIFICATION OF THE SEAT BELT MANUFACTURER FOR SEAT BELT ASSEMBLIES.

DEALERS WILL REPLACE THE FRONT CZS WITH A REDESIGNED SENSOR. THE RECALL BEGAN ON JANUARY 26, 2009. OWNERS IN THE OTHER STATES WILL RECEIVE EXTENDED WARRANTY COVERAGE FOR THE SENSOR TO 10 YEARS. THESE OWNERS WILL BE NOTIFIED OF THE WARRANTY EXTENSION BY MAIL AND WILL RECEIVE A STICKER TO PLACE IN THEIR WARRANTY BOOKLET EXPLAINING THE EXTENDED WARRANTY COVERAGE. Slider Lock Failure - The ACL Workstation slider, a component of the ACL workstation, does not consistently lock into the ACL Workstation Base.
On January 9, 2008, sales representatives were notified via telephone. All consignees were mailed an Urgent: Device Removal notice, dated January 10, 2008. The firm requested that any affected inventory in stock be quarantined and returned to Stryker Endoscopy for destruction. A response form with return postage was included with notification mailed to all consignees.

DEALERS WILL REPLACE THE PANNIER RAIL. ALL MOTORCYCLES HAVE BEEN REPAIRED.

A VEHICLE CRASH COULD OCCUR IF ANOTHER VEHICLE STRIKES THE DPF ON THE ROADWAY.
FERRARA IS WORKING WITH CATERPILLAR TO HAVE THE DPF BRACKETS REPLACED (PLEASE SEE 08E041). OWNERS HAVE BEEN NOTIFIED AND ARRANGEMENTS HAVE BEEN MADE TO HAVE THESE VEHICLES REPAIRED.
