REAR STABILIZER JACKS
IF THE JACKS EXTEND WHILE DRIVING, IT INCREASES THE RISK OF LOSS OF VEHICLE CONTROL LEADING TO A VEHICLE CRASH.
DEALERS WILL REPLACE THE EXTEND/RETRACT SWITCH WITH A SEALED SWITCH SUPPLIED BY KEYSTONE. THIS SERVICE WILL BE PERFORMED FREE OF CHARGE. THE SAFETY RECALL BEGAN ON DECEMBER 11, 2009. ROOF PANEL SEPARATION
IF THERE IS A COMPLETE SEPARATION, THE ROOF PANEL MAY DETACH FROM THE VEHICLE. IF THIS WERE TO OCCUR WHILE THE VEHICLE WAS BEING DRIVEN, IT COULD STRIKE A FOLLOWING VEHICLE AND CAUSE INJURY AND/OR PROPERTY DAMAGE.

INSUFFICIENT STEERING GEAR MOUNTING BOLT TORQUE
THE INSUFFICIENT CLAMP LOAD MAY ALLOW THE FASTENER(S) TO BECOME LOOSE AND FALL AWAY FROM THE VEHICLE RESULTING IN ROAD DEBRIS. IF MULTIPLE FASTENERS WERE TO BECOME LOOSE, THE VEHICLE STEERING MAY WANDER, WHICH COULD RESULT IN A CRASH.

Cressi Ellipse Black MC5 Regulator Recalled for Drowning Hazard
Partial obstruction of the High Pressure (HP) port can produce an inaccurate reading on the pressure gauge, resulting in a slow descent of the needle in the pressure gauge. The inaccurate reading on the gauge poses a drowning hazard to divers.
Consumers should immediately stop using the regulators and return them to a Cressi authorized dive shop for a free repair. Consumers can also return the product to Cressi at One Charles Street, Westwood, N.J. 07675.
Tool Bench Utility Knife Recalled for Laceration Hazard
The utility knife's blade can slide past the blade support during use, posing a laceration hazard to consumers.
Consumers should immediately stop using the recalled utility knife and return it to the store where purchased for a full refund.
Vintage Verandah Patio Fireplace Recalled for Fire Hazard
A paint used to finish the exterior shell may catch fire, posing a fire hazard to consumers.
Consumers should stop using the fireplaces immediately and return them to their local Badcock store or place of purchase.
Biomet Dual Offset Broach Handle, Clamping Style, Left, REF 31-555403. For use in primary hip arthroplasty.
The handle body may fracture or crack at the slot where the locking lever is inserted to clamp down onto the broach.
Biomet notified their distributors by recall letter dated 3/31/09, instructed them to pick up the insturments at the hospitals and to notify the hospital personnel of this recall action in writing.
Xoran MiniCAT System for ENT (server and work station), Xoran Technologies, Ann Arbor, MI. Cone beam CT for sinus, temporal bones, and maxillofacial imaging.
The system may overheat, causing poor image quality, interruption in image acquisition and/or damage to the system. Also, if the system is not allowed to cycle down prior to being restarted after an emergency stop, the x-ray and gantry motions will both resume.
A letter dated 9/17/08 was sent to each account telling them that the PLC would be upgraded. The firm visited each account and made corrections.

THE DAMAGED LPG SUPPLY LINE MAY RESULT IN A PROPANE GAS LEAK AND COULD RESULT IN A FIRE.

One-inch 140X-9 Temperature and Pressure Relief Valves Recalled for Burn Hazard
The relief valve can fail to reduce pressure and avert failure or rupture of the water heater tank and associated valves, posing rupture and burn hazard to consumers.

Promotional Knife Recalled for Laceration Hazard
The knife can fail to lock into its open position, posing a cutting hazard to consumers.
Consumers should stop using the knives immediately and either return the knives to their local 5.11 Tactical retailer or contact 5.11 Tactical directly to arrange for the return and exchange of the knives for a substitute promotional item at no cost to them.
Dive Rite Wings Recalled for Drowning Hazard
The overpressure valve springs in the diving equipment could rust and fail allowing the buoyancy compensator devices to leak, posing a drowning hazard to divers.
Consumers should immediately stop using recalled diving equipment and return it to an authorized distributor or dealer for a free replacement of the overpressure valve spring.
2009-2010 Polaris All-Terrain Vehicles (ATVs) Recalled for Injury Hazard
The front suspension ball joint stem can separate from the steering knuckle and cause the rider to lose steering control, posing a risk of injury or death to riders.
Consumers should immediately stop using the recalled ATVs and contact their local Polaris dealer to determine if your model and VIN number are included in this recall and to schedule a free repair. Polaris is directly contacting registered owners about the recall.
LEOPARD Highchairs Recalled for Choking Hazard
The snap locks used to secure the seat to the frame can break and allow the seat and child to drop through the frame, posing a fall hazard to young children. Detached snap locks can pose a choking hazard to young children.
Consumers should immediately stop using the highchairs and return them to any IKEA store for a full refund.
Nucleus Non-Magnetic Plug for removable magnet implants, REF Z50100, STERILE EO, Cochlear Limited.
Cochlear implant device component was mis-labeled.
On September 28, 2009, each surgeon who received a non-magnetic plug after December 5, 2008 was sent a notification letter via courier. The letter described the mislabeling issue and actions for consignees. Customers were requested to return affected product (and the quantity discarded) to the firm and review patient records to verify the implantation of the non-magnetic plug and not the mislabled product (sterile magnetic). The recalling firm notified its consignees of the problem and the recall by telephone on 10/19/2006. The firm followed this with a written notice on 10/30/2006.
ILLICO Posterior Thoracolumbar Access Retractor, Part Number: 73520 The ILLICO Posterior Thoracolumbar Access Retractor is an instrument that is intended for use in a minimally invasive approach for surgical procedures. The retractor allows retraction of tissue through combination of articulating blades. Device will serve to provide access for surgeons to surgical site.
The recall was initiated after Alphatec Spine received a complaint on the ILLICO Posterior Thoracolumbar access retractor. It was reported that one of the retractor arms in the retractor broke during surgery.
On January 28, 2009 Alphatec Spine contacted their domestic consignees via telephone and the international consignee was contacted via telephone and electronic mail. Alphatec Spine's telephone call consisted of informing the consignees of the product affected, the problem, what actions needed to be taken and what action Alphatec Spine would take to arrange for pick-up of the affected products.
