Roman Shades with Black-out Lining Recalled for Strangulation Hazard
Strangulation can occur when a child places his/her neck between the exposed inner cord and the fabric on the backside of the blind or when a child pulls the cord out and wraps it around his/her neck.
Consumers should immediately stop using these Roman shades and contact Restoration Hardware Baby & Child to receive a free repair kit. All known purchasers have been contacted by the firm. The free repair kit will be available within six weeks.
Hampton Bay Dehumidifiers Recalled for Fire Hazard
An internal component can fail causing the dehumidifier to overheat, posing fire and burn hazards to consumers.
Consumers should immediately stop using the recalled dehumidifiers and contact Home Depot to receive a gift card for the full amount of the purchase price.
Faux Wood Blinds Recalled for Strangulation Hazard
Strangulation can occur when a child places his/her neck between the cords of the pull cord above the breakaway device and the device fails to breakaway.
Consumers should immediately stop using these faux wood blinds and contact American Vintage to schedule an appointment for on-site repair of their blinds.
Precious Moments Angel Christmas Tree Toppers Recalled for Fire Hazard
Undersized wiring can cause the tree topper's switch assembly to overheat and melt posing a fire hazard.
Consumers should immediately stop using the tree toppers and return them to the place of purchase for a full refund.
Boy's Fleece & Flannel Zip Hooded Sweatshirts with Draw Recalled for Strangulation Hazard
The hooded zip sweatshirts have a drawstring through the hood which can pose a strangulation hazard to children.
Consumers should immediately remove the drawstrings from the sweatshirts to eliminate the hazard or return the garment to either the place of purchase or to Jason Evans for a full refund.
CO2 bicycle tire inflators Recalled for Injury Hazard
The pressurized cartridge containing carbon dioxide (CO2) can forcefully separate from the pump head, posing a risk of injury to the consumer.
Consumers should immediately stop using the inflators and return them to Walmart for a full refund.
Action Team Toy Dart Gun Set Recalled for Choking Hazard
If a child places the soft, pliable plastic dart in his/her mouth, he/she is likely to choke/aspirate the dart into her/her throat impairing the child's ability to breathe. If the dart is not immediately removed, brain damage or death can occur.
Consumers should immediately take the recalled dart gun sets away from children and contact OKK Trading to return the toy for a $3 bounty. OKK Trading will provide a free postage paid envelope for consumers to return the toy.

False Positive Results: Product labeling states cross contamination is not an issue for high viral load titer samples, but the firm has determined that a potential sample contamination may not be detected at these levels and could result in false positive results being reported.
Consignees were sent an Urgen Medical Device Correction letter dated 5/14/07 which instructs them to define and establish a lower detection limit, to use UNG to prevent carryover whenever possible, and to confirm low positive results through an independent experiment. A revised device correction letter dated 6/15/07 was subsequently sent to each consignee updating the reason for recall and associated risks.
Stryker Impaction Bur Guard REF 2296-301, 2296-300, 0260-901-300, & 5400-300; Stryker Instruments, Kalamazoo, MI. Intended to be used with impaction Drill. Drill is used for surgical procedures involving precision drilling and cutting of bone and hard tissue. The bur guard covers the nose of the handpiece and the shaft of the cutting accessory to prevent exposure to heat and moving parts, as well as minimize bur whip and handpiece vibration.
Bur guards were distributed without an expiration date, and if they are used after they have expired it could result in the patient or operating room staff experiencing a burn or laceration.
Customers were notified via a Medical Device Recall Notification letter dated 1/27/09 and instructed to immediately check all stock areas or operating room storage and inspect their bur guards for an expiration date. They were also instructed to quarantine any affected product found, to indicate on the Business Reply Form the quantity of affected product on hand, to destroy any affected product found without an expiration date by breaking off at least 3 of the tabs on the bur guard, then dispose of the affected product in normal trash, and complete and return the business reply form. Customers are to fax the Business Reply Form to Stryker Instruments Regulatory Department, 866-521-2760. Upon receipt of the Business Reply Form, a replacement bur guard(s) will be sent for any affected product destroyed.

Moisture ingress into the electronic liquid level indicator may cause inaccurate liquid oxygen level readings. The oxygen could run out without the patient knowledge.
A Technical Service Bulletin was sent to consignees on/about October 23, 2007 to notify customers of the potential issue. IF THE DRIVE PIN BECOMES DISENGAGED ON THE BRAKE PEDAL ASSEMBLY, IT COULD RESULT IN A LOSS OF VEHICLE BRAKING FUNCTION AND/OR CAUSE THE BRAKE PEDAL TO INTERFERE WITH THE THROTTLE PEDAL FUNCTION. EITHER CONDITION COULD RESULT IN A VEHICLE CRASH.
