CROWNZZ™ Candleholders Recalled for Fire Hazard
The candleholders do not allow for the safe burning of a candle, which poses a fire hazard to consumers.
Return the candleholders to the store where purchased for a refund.
EBI VueCath Endoscopic Spinal System
Non sterile device is labeled sterile.
The firm notified their customers via a field correction letter dated June 17, 2003. Instructions given are to sterilize the unit prior to use. A EBI field representative will be visiting to replace the boxes.

Kits had parts in them that did not belong in the kit.
Customers have been informed to discard kits with the incorrect adjustors in them by fax delivered on 7/31/2003.

Laser marking is wrong on bone screws, says 40 and is 45 mm long.
Customers were notified by fax in a letter dated May 19, 2003. All customers have been notified.
Jack-In-the-Box toys Recalled for Choking Hazard
A spring mechanism attached to the lid can break and detach from the toy, posing a choking hazard to young children.
Call Avon for information on how to receive instructions to remove the spring mechanism. Known purchasers are being sent direct mail notification of this recall.

John Deere 4000 Ten Series Compact Utility Tractors Recalled for Injury Hazard
An internal failure may occur within the Hydrostatic Transmission preventing the transmission from returning to neutral after the pedals are released. This can allow unexpected movement or cause the tractor to continue moving, creating a possible risk of injury to the operator or a bystander.
Consumers should stop using their tractors immediately and contact a John Deere dealer for a free repair.
Crate and Barrel "Hanukkah Menorah" Recalled for Fire Hazard
The acrylic base of the Hanukkah menorah can ignite if the candles are allowed to burn completely, creating a fire hazard.
Do not ignite any candles in this "Hanukkah Menorah." Stop using the menorah immediately and return to the store for a full refund. You may mail the menorah to Crate and Barrel and receive a refund check.
Fix pins associated with Stealth Station Treatment Guidance Platform
Surgical devices intended for single use were not labeled for single use.
Consignees were notified by letter on 11/01/2003.
Hill-Rom Newborn bassinet; model P247
The caster/wheel may come off of the bassinet, causing the bassinet to tip.
Customer notification letters dated June 20, 2003 were sent to each customer. Customers are asked to verify the security of the casters to the base and to visually check for loose or missing caster screws, to remove any bassinets from service until repaired, and that Hill-Rom will visit each customer to replace the caster screws on all units.
MAZDA/FUEL SENDER UNIT
A FIRE COULD OCCUR IN THE PRESENCE OF AN IGNITION SOURCE.
DEALERS WILL INSPECT THE TOP OF THE FUEL TANK, AND IF NECESSARY, REPAIR OR REPLACE IT. OWNER NOTIFICATION BEGAN FEBRUARY 9, 2004. OWNERS SHOULD CONTACT MAZDA AT 1-800-222-5500.

APPLICABLE CHILD SEATS WOULD NOT BE ABLE TO UTILIZE THESE SUPPLEMENTAL ANCHORAGES. IN THE EVENT OF A CRASH, THE SEAT OCCUPANT MAY NOT BE PROPERLY RESTRAINED, INCREASING THE RISK OF INJURY.

GM/STEERING GEAR BEARINGS
IF THIS OCCURS, THE PINION SHAFT CAN BE FORCED UPWARD DURING LEFT TURNS AND BACK DOWN AS THE STEERING WHEEL IS MOVED BACK AND TO THE RIGHT. IF THE PINION SHAFT MOVES FURTHER, THE DRIVER WILL NEED TO EXERT MORE EFFORT TO TURN THE STEERING WHEEL, SIMILAR TO A VEHICLE WITHOUT POWER ASSISTED STEERING. IF THE PINION SHAFT MOVES EVEN FURTHER, THE DRIVER WILL REQUIRE MUCH HIGHER EFFORT TO TURN LEFT AND MAY NOT BE ABLE TO TURN THE WHEEL AS MUCH AS INTENDED. WITH THE MAXIMUM PINION SHAFT MOVEMENT, WHICH REQUIRES INTERNAL GEAR COMPONENT DAMAGE, THE DRIVER CAN ENCOUNTER HIGH RESISTANCE TO TURNING LEFT, FOLLOWED BY UNINTENDED POWER ASSIST TO THE RIGHT. IN ANY OF THESE CONDITIONS, A CRASH COULD OCCUR.
DEALERS WILL INSTALL A NEW LOWER PINION BEARING UNLESS INSPECT OF THE EXISTING BEARING INDICATES THAT REPLACEMENT OF THE GEAR ASSEMBLY IS NECESSARY. OWNER NOTIFICATION BEGAN ON FEBRUARY 5, 2004. AMERICAN IRONHORSE/BRAKE ROTOR-CARRIER
THIS CAN CAUSE A PARTIAL LOSS OF THE BRAKE SYSTEM OR THE POTENTIAL FOR ROTOR-CARRIER DETACHMENT, WHICH COULD RESULT IN A CRASH.

WELLCO/SPRING BRAKE CONTROL VALVE
FAILURE OF THE VALVE IN AN EMPTY OR LIGHTLY LOADED TRAILER WHILE THE VEHICLE IS IN MOTION COULD RESULT IN A HARD APPLICATION OF THE SPRING BRAKES, POSSIBLY LOCKING UP THE TRAILER WHEELS AND RESULTING IN A VEHICLE CRASH.
SEALCO WILL PROVIDE A REPAIR KIT AND INSTALLATION INSTRUCTIONS TO CUSTOMERS FREE OF CHARGE.
